Evidence required for a Blue Badge if you have a terminal illness
Delayed application
Your application will be delayed if you do not provide all of the required evidence.
When you apply for a Blue Badge, we will ask you to provide a completed SR1 form that confirms your life expectancy, or the life expectancy of the person you support.
We cannot accept:
- medical or clinical letters
- incomplete SR1 forms
