Only complete and submit the review form if you have been requested to do so by the Local Authority Benefits Service.
If you do not submit your completed review form with supporting evidence (as detailed within the form) within 1 month of the date of the request, the Local Authority Benefits Service will need to consider why, and possibly stop your entitlement.
