Overview
If you're going into hospital there are some things you may need to think about to prepare for your stay and coming home.
If you currently get support services from us, get in touch with us so that we can pause these services until you come home.
Going into hospital
In the event of an accident or a sudden change in your health condition, it may be necessary to visit the hospital.
Medical doctors and other healthcare professionals will assess your condition to determine the appropriate diagnosis and required assistance. They will communicate this information to you and your family.
It may be determined that admission to hospital is necessary for further tests or treatment.
If an overnight stay is not required, you will be discharged home.
If you cannot make decisions about a hospital stay
Professionals may undertake a Mental Capacity Assessment to support your discharge planning.
If you cannot make a particular decision, a decision will be made in your best interests by consulting people who are important to you. You will still be involved in the decision as much as possible.
If you do not have anyone to help you, an advocate will make sure that your views and wishes are heard.
Assessment in the Hospital
If you need to stay in a hospital, you will be assessed by professionals. These professionals may include:
- Occupational therapists
- Physiotherapists
- Social workers
They assist you in returning home safely by:
- Helping you to regain strength
- Identifying necessary equipment for your home
- Arranging short term support
Their goal is to help you achieve maximum independence.
