Overview

The Adult Learning Disability Service is dedicated to empowering adults aged 18 and over with a diagnosed learning disability.

We are an integrated team of health and social care workers from BCP Council and Dorset HealthCare University NHS Foundation Trust who collaborates to support individuals in leading full and independent lives.

Our team includes social care workers, learning disability community nurses, occupational therapists, and physiotherapists. We also have strong connections with consultant psychiatrists, psychologists, and speech and language therapists.

Our services are accessible across Bournemouth, Christchurch and Poole.

What we do

We conduct person-centred assessments to identify each individual's strengths, needs, and goals. Our support can be provided through various means, such as:

  • engaging with existing community groups
  • friends or family
  • participating in adult education
  • utilizing commissioned services and direct payments
  • assistance from our healthcare staff

We work together to:

  • empower you to reach your full potential in all areas of your life
  • support your carers and others in assisting you
  • provide health advice to maintain your well-being and teach essential life skills for greater independence
  • help you find a safe and supportive living environment
  • offer guidance and assistance in obtaining equipment to ensure safe mobility within your home

Adults aged 18 and over with a diagnosed learning disability, along with their family members, advocates, and professional colleagues can refer to our team through the Adult Social Care Contact Centre.
