From 1 April 2024, you can cancel your care using the online form below.

Please consider your needs before you cancel your care. If your current care package isn't working well for you, we hope that you will contact your worker first to see if there are any ways we can offer something different.

If you do cancel your care, we cannot guarantee that you will be able to get the exact same package if you applied for care again later on.

 

You will need to keep paying for your care for up to 4 weeks after we confirm that the care package has been cancelled.

Once we receive your cancellation request, we will get in contact to discuss the next steps.
