Intermediate care

The intermediate care team can help you avoid admission to hospital when you’re unwell. If you do have to go into hospital, we'll work with hospital staff to arrange your discharge and rehabilitation plans.

How we work

We work with you for a short period of time, in your home or at a rehabilitation unit and help you with things like:

  • improving your confidence
  • living safely and independently at home
  • preventing admissions to hospitals or care homes

Charges

The team’s assessments and services are free. There may be charges for other services.

If we arrange other services, we’ll check your finances to see how much you need to pay. We’ll also check that you’re getting all the benefits you’re entitled to.

Applying for help

If you’re in hospital, your nurses, therapists or doctors can help you decide if our services are right for you. If you live at home, you should discuss your needs with a GP, district nurse or social worker.

Mobile night care service

The mobile night care service offers short-term care at night for people who live at home. This is covered between 10pm and 7am, on a planned basis or for emergencies. It normally lasts for a maximum of 6 weeks.

Coastal Lodge

Coastal Lodge is a CQC registered care home in Bournemouth that provides short term rehabilitation and reablement care.

Coastal Lodge is mainly, but not solely, for older people, providing care for dementia, mental health, sensory impairment, substance misuse and eating disorders, as well as end of life care.

You can use Coastal Lodge if:

  • you've been discharged from hospital but not ready to return home
  • you're currently at home in the community but need short term care

Respite care or more permanent residential care is offered at Coastal Lodge, depending on the availability of facilities.
